Summary Plan Descriptions (SPDs) provide you with a general overview of your plan coverage. Click on the Plan Name below to open a copy of your SPD.
The purpose of an SPD is to explain the plan in a way that can be understood more easily than the formal language of the plan document. All final eligibility and payment decisions, however, must be made according to the language of the actual plan documents, which is the controlling document over the plan.
